You can locate Fair Pricing Policy information for hospitals in California by entering the
Hospital Name, address, city, or ZIP code. The following are instructions for quick searches:

General Search
You can search for hospitals by entering one of the following search criteria:
- Hospital name
- Street address
- City
- ZIP code
Please note: this search box will search hospital names, street
addresses, cities, and ZIP codes together. For example, if you enter part of a hospital
name such as "Santa", search results will yield hospitals where Santa is part of the
hospital name, street address, or city.

Quick Tips:
- In general, do not enter street type such as street or boulevard as it may be
too specific and may not yield any results.
- If you are searching on a lettered street (e.g., J Street), enter address as
"J St" (i.e. abbreviate street type and do not add punctuation). Entering
only "J" may yield too many results. Entering street type as "Street" will
not yield any results.
- It is not recommended that you enter an address number as it may be too
specific and not yield any results.
Search by Radius (Distance and Location)
A radius search allows you to search for hospitals in a particular
area of California. For example, you can locate all facilities in the San Francisco Bay Area.
To complete a radius search follow these steps and click Search:
- Select a "Distance" and enter information in the "From Location" field. You must enter
information in both fields to yield search results.
- To narrow search parameters, select a smaller distance from the desired location.
Example: Distance = 10 miles, From Location: Sacramento.
- Enter a Location such as street name, city and/or zip code. To yield more specific
results, enter only one location option (e.g., only city or zip code).
Viewing Search Results
Search results are displayed showing hospital name, city, hospital type, emergency room
availability, Federal Poverty Level (FPL), and the effective date of the submitted fair pricing policy.
- You can show hospital addresses by clicking on the plus sign ("+") located next to
Hospital Name. Clicking on the minus sign ("-") will hide addresses.
- You can sort each column in ascending or descending order by clicking on the headings.
- Emergency Room availability is shown by a check. It does not indicate the emergency
room's level or whether it's a designated trauma center.
- The FPL column shows the maximum Federal Poverty Level (FPL) for which a payment
discount is provided (See Show FPL Chart below).
- The Effective Date column displays the effective date of the hospital's fair
pricing policy. "Not Yet Posted" means that the policy has not yet been submitted
or has been submitted and is currently under review before being released.
Downloading Results
You can download the results of your search or download information for all
hospitals. You must have Excel 2003 or higher to download results. To download:
- Click on the blue Download Results or
Download all Hospitals link above the results table.
- You will be asked to confirm downloading of results. Click OK to download.
- You can either open results in a new window or click Save to save the results.
- If you click Open, Excel will launch and results will be populated into a spreadsheet.
NOTE: If you are downloading all hospitals, you may experience slow performance if your machine
does not have sufficient memory.
Viewing Detailed Information
You can view detailed information for a hospital:

- Search for a hospital.
- Click the hyperlink for the desired hospital in the search results list. Alternatively, you can
click on the red marker on the map and click View Fair Pricing Policy for a desired hospital.
- A new page with the hospital details will be displayed. From this page, you can view general
information such as the hospital's address and contact information. You can also see summary
information regarding its fair pricing policy and retrieve the hospital's detailed fair
pricing policy and application.
- Click the more... hyperlink to view
the license information for the hospital selected. A new window with licensing information is provided.
(NOTE: You must be using Microsoft's Internet Explorer version 5 or greater to view this page).
- To view the policy and/or application submitted for the hospital selected, click
Policy File
or Application File
links.
NOTE: To open and view the hospital's submitted fair pricing policy and application form,
you must have Adobe Acrobat Reader. If you do not have Adobe Acrobat Reader, click on the
"Get Adobe Reader" logo. You will be taken to the Adobe website where you can download a
free copy of Adobe Reader.
- When you arrive at the Hospital Detail page, the most recent information is provided. If there are
older submissions (e.g. from previous years or modifications to the current year), you may select
them from the Policy Effective Date (Date Posted) drop down at the top of the page.
- **If you have questions about your eligibility for a payment discount, please call the Hospital
Contact listed on the Hospital Detail page**.
Show FPL Chart
Clicking on the "Show FPL Chart" displays the current Federal Poverty Level (FPL) chart. The chart shows how household size and annual income compare to a range of
percentages of the Federal poverty level (as established by
the U.S. Department of Health and Human Services). For more
information about the federal poverty guidelines, go to:

To close the char, click the "Hide FPL Chart" link.

Using the Map
After entering search criteria, the map will pinpoint the location(s) of your search results. The markers that display can be clicked to open up detailed information
on the hospitals.

- You can use the zoom controls to see more or less of the map.
- You can move the map in any direction by clicking on the arrows.
- Clicking on the red pin will display the name and address of the hospital.
- Clicking on View Details will open another page which displays general information about a
hospital along with its fair pricing policy and application form.
- Clicking on the X in the hospital detail will close the bubble.
Using Map Using Search Criteria
Locate a hospital by entering search criteria and selecting the hospital from the map.

- Gray markers are provided to indicate surrounding hospitals
that were not part of your original search criteria.
- Clicking on the marker will display an information bubble
with the hospital name and address.
- Clicking on View Fair Pricing Policy link in the information
bubble will open another page which displays general information
about the hospital, along with its fair pricing policy and
application form.
- Clicking on the X in the hospital detail will close the bubble.
